* 1 Permit for burial & 1 Death Certificate and Sales tax

* Standard metal caskets ($2,399 value)

* 1- day chapel use for visitation (2PM - 6PM) / 1 - day chapel use for service includes Saturday (3hr. max) Officiant can be provided if needed.
